Only for presentation person: REQUEST FOR HELP FOR FUNERAL EXPENSES
If it does not authorize Mutuality to consult your identity: Photocopy of ID CARD.
Certification of the Civil registry supporting deceased's death (original or attested photocopy) or attested photocopy of the family record book containing the death.
Original invoice (with the data and requirements of article 6 of the R.D. 1496/2003) of burial of the deceased and funeral issued on behalf of the applicant, if the right of the person who has taken charge of the expenses of bereavement.
To request this provision must be discharged as the mutual scheme. The General identification will be achieved through the system Cl@ve so you will need to be discharged in that system or have dni-electrÃ³nico or one of the digital certificates admitted .

The application form and associated documents may be submitted at the central or provincial branches of the society in General, or in any other places under article 16 (4) of law 39/2015, of Common Administrative Procedure of the public authorities.
