Medical certificate stating the coeliac condition suffered by the beneficiary.

Certificates of registration of all members of the family unit.

If it does not authorize Mutuality to consult data: attested photocopy of personal income tax returns of family unity, of the period immediately prior to the request for help.

If it does not authorize Mutuality to consult data: statement responsible of pensions or benefits exempt or not subject to taxation from INCOME TAX .
IMPORTANT:
You have to keep the originals of the invoices and other original documents probative value for a period of four years of the date on which the benefit.
