Certificación de la Unidad Pagadora en el que consten las retribuciones básicas integras correspondientes al mes en que se produzca la jubilación.

Certificación de la correspondiente Unidad de Personal, acreditativa de la situación administrativa (sólo es necesario si no apareciera reflejada en la resolución de jubilación).

Si no autoriza a la Mutualidad General Judicial a consultar sus datos de identidad: fotocopia del DNI.
Para solicitar esta prestación debe estar dado de alta como mutualista titular. La identificación se realizará mediante the system Cl@ve
